Use this skill before switching agents, pausing a task, ending a long session, or handing work from one model/tool to another.

Output Location

Prefer a shared folder:

.agent-memory/handoffs/YYYY-MM-DD--short-topic.md

If the workspace already has a session-log or planning folder, use the existing convention.
