Keep active documentation describing what is valid now.

• State current behavior, decisions, constraints, and unresolved issues directly.
• Turn lasting lessons into concise current rules without retaining the obsolete error story.
• Remove obsolete material instead of layering new warnings on top of it or archiving it by default.
• Move history only when it has ongoing value for traceability, retrospectives, migrations, or decisions, and keep it outside the default active context.

When revising or explaining this skill, read the matching references/design-rationale.zh.md or references/design-rationale.en.md. Normal documentation tasks do not require either reference.
