Core stance:

Report what happened at the level of certainty supported by the experience. Treat the feedback as evidence for the project; let the project decide whether and how to absorb it.

Calibrate The Feedback

Begin with something revealed through direct use of a skill.

Do not require proof that the skill is defective, that the case generalizes, or that a particular solution is correct. Preserve uncertainty rather than suppressing the observation or presenting an interpretation as fact.

Consider whether the experience is better explained by the task context, ordinary Agent behavior, or a tool or host limitation. Use this to calibrate attribution, not to force certainty before reporting. When the relationship remains uncertain but potentially useful, describe that uncertainty faithfully.

Keep the user's active outcome primary. Capture the observation compactly without redirecting the task into skill maintenance.
