Addressing PR comments

An item is an unresolved inline review thread (anchored to file and lines; only threads resolve), a review summary with a body, or a conversation comment; summaries and conversation comments are PR-level.

By default, carry out each item's recommendation without asking; items that fail the apply bar pause. Named items or options ("decline 4") replace their recommendation. Asked only for recommendations, or under a watching-authored-prs present-then-stop grant, add the recommend-only checks.
