Docs Maintain

Update the canonical documentation that changed behavior makes inaccurate or incomplete.

Procedure

1. Read docs/index.md, then only the documents routed for the affected area.
2. Verify the change against code, tests, configuration, or an explicit user decision.
3. Update the narrowest canonical file. Create docs/modules/<subject>.md when a bounded module would otherwise enlarge a macro document.
4. Update docs/index.md when adding, moving, or retiring a document.
5. Remove or replace stale guidance in the same change. Do not leave parallel active descriptions.
6. If the resolved shared harness repeats canonical content, retain only a pointer plus any still-provisional context.
