• Empty alt: ![](...)
• Filename as alt: ![img_1234.jpg](...)
• Generic alt: ![image](...), ![screenshot](...), ![photo](...)
• Alt that is just punctuation or a single character

Auto-fix: No - always flag and suggest, require human approval.
