Phase Outline Skill

Role: Two-lane workflow skill for creating solution outlines. Routes on the planning lane resolved by the phase-1-init lane router (status.metadata.planning_lane).

Prerequisite: Request must be refined (phase-2-refine completed); the planning lane (light/deep) is set by the phase-1-init lane router.
