Turn the task into one governing implementation contract: what must change, what must remain intact, how the selected design works, and how to verify the result. Write for an implementer who has the spec and its identified sources but none of this conversation. Deliver the specification without implementing it.

Recover the current task and its evidence

On every invocation or continuation, reread the task, resolved user decisions, repository instructions, and research sources. For an existing spec, read its complete current text and required linked parts, including edits made since the previous invocation. Read supplied feedback in full. Use memory and old reports to locate sources, never to reconstruct their present contents.
