Use t3code-thread to create or message the target thread.

The receiving thread starts with no context. The handoff prompt must be complete enough to act without this conversation.

Resolve:

• New thread or existing thread id.
• Same project or another project.
• Worktree requirement. Default to a new worktree for implementation handoffs unless the user explicitly wants the existing checkout.
• Whether the task is implementation, investigation-only, review, or planning.
• Provider/model choice if the user specified one.
