Docforge Revise

Slash command: /docforge-revise. Thin entrypoint into the docforge skill — this skill has no runtime of its own. It requires the docforge skill to be installed and loads its shared cartridge: ../docforge/_shared/README.md.

Migration is unconditional and needs no confirmation — every invocation upgrades the manifest and moves provenance automatically (../docforge/_shared/workflows/validation.md "Manifest and provenance"). Writing or rewriting any document always stops and asks first — never on silent defaults.
