Set up an OKF Wiki vault

Ask the five questions in references/questionnaire.md, then write their answers to a temporary JSON file and run scripts/okf_setup.py plan. Show every proposed write and every untouched path.

Git is mandatory. If the target is not already version controlled, ask for Git initialization consent before final approval. A refusal aborts without calling apply.

Never write before explicit approval. For updates, present every conflict diff and obtain one keep, apply, or alongside decision per path. For offered template updates, pass --accept-updates all or an explicit comma-separated path list. Run apply only with those explicit decisions and --initialize-git when the plan requires it. Never create a commit.

For an invalid existing vault, report the validator findings and direct the user to okf-wiki-lint; do not repair it automatically.
