Scope

Getting from "someone asked for a thing" to "a plan an executor can follow and a reviewer can check": clarification, option selection and its record, decomposition, the shape of the plan document, and keeping the plan honest while it is executed.

The cheapest plan is often no document. A change whose flow already exists here and can be read end to end needs a stated intent and agreement, not a file. Reach for a document when the work spans subsystems, outlives one session, or will be executed by someone other than its author.
