Move issues on tracker through small state machine of triage roles.

When repo treats external pull requests as request surface (see tracker config), triage covers them too: a PR is an issue with attached code -- same roles, same states, same machine, with deltas marked "for a PR" below. Resolve a bare #42 to an issue or PR per tracker.

When explore codebase, use project domain glossary so titles, comments, agent briefs match project language. Respect ADRs in area touched. Use /read-the-damn-docs for external/current behavior, /plan-arbiter for competing fix plans, and /visual-plan for large epics.
