• Outcome: A findings list grouped by file, every item in file:line form.
• Done when: Each finding points at a concrete line; files with no findings are listed as ✓ pass.
• Evidence: The quoted line content behind each finding.
• Authorization: Read and report only — do not edit the reviewed files unless the user asks for fixes.
