Per Entry

• Decision — what was decided, plainly
• Context — the situation that prompted it
• Options considered — what else was on the table
• Rationale — why this one
• Decider & date — who owned it, when
• Revisit-by — when (or whether) to reconsider

Method

1. Log decisions at the moment they're made, not later
2. Record the reasoning, not just the outcome — future-you needs the why
3. Note what was rejected, so it isn't re-proposed
4. Keep it searchable and linked from the program hub

Best Practices

• "We discussed it" is not a decision — capture the call
• A decision without a decider isn't one
• Link decisions to the work they govern
