Per Risk

• Description — what might happen and why
• Likelihood x impact — scored, giving a severity
• Owner — who watches and acts
• Response — avoid, mitigate, transfer, or accept
• Trigger — the early signal it's materialising

Method

1. Brainstorm risks with the team, not alone
2. Score likelihood and impact on the same scale
3. Focus energy on high-severity risks; log the rest
4. Assign an owner and a concrete response to each top risk
5. Review and re-score at every cycle

Best Practices

• A risk with no owner is decoration
• Distinguish risks (might happen) from issues (have happened)
• Retire risks that have passed; add new ones as scope shifts
