P1 Extract hard constraint: load prompts/extract-claims.md + references/schema.md BEFORE producing any claim; claim sets are assembled ONLY by scripts/build-claims.py (minimal fields via stdin); on failure re-emit ONLY the failed claims — never rewrite the whole set.
