Startup version evidence

Before the first PR-state or file mutation, follow ../dstack-core/references/SKILL-VERSION.md for gh-pr-review. After read-only PR inspection, capture the exact one-line output and record it in the PR review state before mutation. A stale result warns with npx skills update; unavailable records that no freshness claim was made and does not block offline work.
