NO FIXES WITHOUT ROOT CAUSE INVESTIGATION FIRST
NO INVESTIGATION WITHOUT CONTEXT RECALL FIRST

If you haven't completed Phase 0, you cannot proceed to Phase 1. If you haven't completed Phase 1, you cannot propose fixes.
