Fix PR Feedback

Address PR feedback with automated discovery tracking and iterative fixes.

Purpose

Handles the iterative refinement loop after PR creation:

• Read PR comments (human reviews, bot comments)
• Check CI status (failing tests, linting errors)
• Detect merge conflicts
• Create child Beads issues for each discovery
• Fix issues or ask user for guidance
• Commit fixes with proper Feature-Key trailers
• Push updates to same branch
