PR template

Give a repository one pull-request template — the file tailrocks-create-pr and tailrocks-refresh-pr read by default. Preserve the exact spelling and location of a sole existing GitHub-supported template; only an absent layout creates .github/PULL_REQUEST_TEMPLATE.md. The starting shape is references/PULL_REQUEST_TEMPLATE.md; the job is tailoring it to what this repository actually is, from evidence: its structure, its real gates, and how its merged PRs are actually written. Resolve every relative link in this file against the directory containing this SKILL.md, never the plugin skills root.
