Reconcile

Restore the truth to an item that has been executed against: every status in roadmap/<slug>/plan/README.md re-earned by a command run now, the newest verification round folded into what is left, and the item's status brought in line with what actually happened. Run when a goal loop finishes or stalls, after a verification round lands, or before resuming an item that sat.

The loop is plan → execute → record feedback → prove → reconcile → repeat. Reconcile makes each round cheap: it marks off what is genuinely done and rewrites the item's ## Remaining from evidence, so the next round reads a short list. When nothing is left open, that pass ends the loop — the item reaches DONE and leaves the tree.
